Care Management

Experienced RN case managers specialize in the care of children with acute and chronic healthcare needs including those who are dependent on various forms of technology. Our case managers perform skilled assessment, planning, monitoring, and evaluation of your child’s physical and developmental needs. Each case manager works with the hospital discharge team to ensure a smooth transition from hospital to home. We coordinate care and advocate for your child’s healthcare needs by communicating with your primary and specialty physicians; all service providers including pharmacy; medical equipment company; physical, occupational, and speech therapists; and others.

Continuous Shift Care
|
When ordered by a physician, families may request shift care by nurses and/or home health aides up to and including 24 hours a day.

Well-Baby, Well-Mom Visits
|
| Our family-centered approach offers experienced RNs who promote the postpartum health and wellness of the mother and baby. Our maternal/infant program includes follow-up for postpartum complications, high-risk and newborn infant care, assessment of vital signs, and nutritional needs assessment. |
